Health subsidies for Northwestern University graduate students vary according to the school and program to which the student belongs. The subsidy to be provided by The Graduate School is calculated at either 100%, 65% or 50% of the cost for the health insurance plan.

The following Payroll Deduction Authorization Templates are appropriate for students at the 50% subsidy level who wish to deduct their insurance expenses from the stipend over a 9-month or 12-month period. Select the appropriate template for your circumstances, complete it, and submit to the appropriate Office of Student Accounts. Blank Payroll Deduction Authorization forms are available as well as links for 9 months or 12 months.  The completed form must be submitted by September 8, 2008, to the Office of Student Accounts on either the Evanston or Chicago campus in order for the first deduction to be taken from your September paycheck. Subsequently, the form must be submitted by the 8^th of each month for processing by the end of the month.
